A flu epidemic complicates the holidays in Poplar as medical staff in addition to residents are felled by the illness, while a prisoner escape frightens the community.

Poplar is hosting a "Mother of the Year" competition, with plenty of candidates, from Shelagh to Violet to someone reunited with an adopted-away son to less obvious choices.

Someone from Joyce's past resurfaces, threatening her new life, while Trixie and Matthew face a rocky future and Rosalind helps a young woman with a horrible secret.

Matthew finally reveals his financial woes to Trixie, right as she was expecting some big lifestyle changes. The other midwives try to help a recently arrived mother and two boys found in a church. 

A young couple underestimates their ability to be responsible parents, but they are put to the test by a medical issue in both their daughters. The Turners face their own trouble with one of their children. 

A distraction at home leads Trixie to rush while working with a patient with the pupil midwife Joyce, while other midwives help a Nigerian family with tuberculosis.

Violet is running for mayor with the support of her family and friends, but a medical emergency threatens to derail her life first.

Cyril and the midwives help a woman whose husband has left her with one child and another on the way and whose government housing is infected with mold.

A new set of midwife pupils arrives in Poplar and one of them is immediately thrust into danger, while the other midwives aid a mother with cerebral palsy. 

The midwives and their friends all pitch in to recreate a cherished Christmas from the depressed Sister Monica Joan's childhood, while also delivering babies and helping an isolated loner.

Almost everything that can go wrong before Matthew and Trixie's wedding does, but they luckily have a caring group of midwives and a supportive community to usher everything through to the end.
